While we are contracted with the following insurance networks, we may not be contracted with certain products offered by the plan. Please contact your health plan directly to verify coverage specific to your policy.

Please note:
If your insurance is not listed, our team may be able to request a Single-Case Agreement (SCA), which could allow your treatment to be covered at in-network benefits. SCAs are reviewed by your insurance provider on a case-by-case basis and are not guaranteed.
If we are unable to accept your insurance, we offer cash-pay rates and can provide a superbill for you to submit to your insurance for possible out-of-network reimbursement. We also accept HSA and FSA cards for payment.
