Please note:
If your insurance is not listed, our team may be able to request a Single-Case Agreement (SCA), which could allow your treatment to be covered at in-network benefits. SCAs are reviewed by your insurance provider on a case-by-case basis and are not guaranteed.
If we are unable to accept your insurance, we offer cash-pay rates and can provide a superbill for you to submit to your insurance for possible out-of-network reimbursement. We also accept HSA and FSA cards for payment.
